2 Errors on vote form view

I found 2 errors in the file "component/com_hikashop/view/vote/tmpl/form.php"

1. The "div" in line 30 does not close properly if "if statement" in line 32 is false
2. the tag "input" at line 95 is not closed properly, missing "/>"
