Order history not showing

I have hikashop 2.1 running on Joomla 2.5 at the domain www.dankhaus.com , and the orders from customers are not showing up on the backend admin panel. I'm positive this feature was working when I first deployed the website, but now it seems to have stopped. There are no orders showing in the reports, and a csv export shows no data as well (image attached). There have been several orders recently and we have no access to the order information. The emails with the order information generated from hikashop are also not working, so we can't see anything about them. Any suggestions for troubleshooting this?

It sounds like you have a database error.
Could you turn on the debug mode of the joomla configuration and try again ? Do you get an error at the end of the checkout (after a successful payment) ?

Attached an image that shows an error about a duplicate transaction was submitted. The owner of the website also said they were having similar problems. I had debug mode on and it didn't show any errors, but I can attach the queries it was showing if you'd like - please let me know.

So from what you're saying it sounds like the problem is simply that the order ids are already reserved in your payment gateway account and thus when HikaShop sends the payment request it is rejected and thus no payment is made and no order is created.
So it's a problem similar to this one:

This one is for PayPal but you can try to do the second solution (with the mysql query) or contact your payment gateway support for help on what needs to be changed in your payment gateway user account to avoid that problem.

I tried the query option but that didn't solve the problem. I realized no billing address was set, so I added that. Now, when you try to create a new billing address, the fields are blank (image attached).

I spoke with my payment gateway provider (authorize.net) and they said the issue is they are not receiving a name and billing address. I think if this billing address issue is fixed, it may start working properly. How do I fix it so that you can enter a billing address?

If you can't see any address fields, it's probably that you deactivated them via the menu Display>Custom fields. So go there and make sure that they are activated.
