Using a view override (orderserial | show_email_notification_html)

2. Yes but it will mean that you create variants for your product and each variant will use a different pack.
Then you have to create several "attach serial" configuration for each pack ; using different images (layouts).
By selecting a variant, the customer will choose a specific design.
Otherwise it might be possible to use an item custom field but it will also require to modify (or duplicate) the plugin "attach serial" in order to manage this new custom feature.

3. Yes ; with variants or options (but I'm less confident in this last one).

4. Yes ; using an item custom field and the "donation" plugin.
It will allow you to create a dynamic price product.
