Customer ability to add notes per item @ checkout?

Is there a way to give the customer the ability to add special notes/instructions for an item in their cart? For example, say they want one book signed, and another book unsigned.

Ideally, I would like to give them the ability to have a (1)checkbox of options and (2)specific instructions PER ITEM.

You should be able to do this with product custom fields, although the shopper will be asked on the product page, not the checkout. There are also order custom fields, but they aren't necessarily per item like the product fields.

Product custom fields allow you to add information to the products from the back end edition page. That's useful for when you want to add information to products like for example an ISBN when selling book.

For information that the customer is supposed to enter, you need custom fields of the table "item" which are available in the Business edition.
