after add something to cart and click "send(Далее)" button i have something like that

Edit the order admin notification via the menu System->Emails and remove the line:
echo "\r\n".$fieldsClass->trans($oneExtraField->field_realname).' : '.$fieldsClass->show($oneExtraField,$data->$fieldName);
in the text version of the email, and that will solve the problem.
