Please advice: how to get this type of form

Hello,

We want to change one of our fabrik form and to move it to Hika. The existing form have some options which i don't know if are available in hika. For example, there is an option in fabrik called repeat group. That mean i can repeat a group of fields so user will be able to complete multiple information and submit as once.

I don't know how to explain better, so please take a look at attached image.

I guess i can do that only in Enterprise or Business version, because i can't find a way to show custom fields anywhere else except address area

If i don't ask too much: can you advice me for something more simple: how to add a text field (below "Quantity" field) in product listing page? All i need is a text field to let user to add a phone number and record it together with options he choosed from characteristics list.
