Registration form issue

I have an issue with the registration form

Its showing only an email entry, no password or anything??

How do fix it?

You should check the way you configured your checkout through the "Login & Registration" tab of the page "Hikashop->System->Configuration->Checkout", and also check if you have published your custom fields for the front-end via "Hikashop->Display->Custom fields".
