How to create simple check box in cart

Hi, after checking out, in second level of payment, there is a checkbox link for term and services. I want to add one more check box below to that check box called "`do you want to receive tips from sales". If it is checked then admin should be notified that user want to receive emails. How can i do that?

If its not possible on that place then any where in cart or checkout area would be fine. Please help.

The solution will just be to use some Order custom fields, and also to place the "Fields" step just after term and services through your checkout workflow via "Hikasop->System->Configuration->Checkout".
