Shipping and Invoice

I have two question.

First, I am trying to do a manual shipping setup with flat rate plus percentage. The problem I am having is that it doesn't work all the time for me. If a customer orders one item, it seems work ok, most of time. If they add anther items, it add shipping, again most of the time it works. If the customer changes their order in the shopping cart, deletes a product or increases or decreases the number of products of the same product, then when the product is refreshed, the shipping fees are gone altogether. The price of the product changes but the shopping fees disappear. I am not sure what I am doing wrong to cause this, but I can't seem to get it right.

Second, and I have found this question and some answers here, but I am not sure I know how to do this. That is, to remove the VAT from the customers invoice.

Can you give us more information about this :

The price of the product changes but the shopping fees disappear. I am not sure what I am doing wrong to cause this, but I can't seem to get it right.

Also, the best solution will probably be to show us through some screenshots for example :
- What do you exactly want
- What you have
- How you configured your shipping method/product to have it.

Sure, let me try to clarify it.

I am using the manual flat rate shipping, set for $5.00.

Customer buys one product, goes to checkout. It shows $5.00 shipping fee.
Customer changes mind about the quality at the cart level, using the plus symbol to add one more.
Refreshes cart with the refresh symbol next to the plus or minus symbol, still in the checkout cart.
After adding another product, from 1 to 2 and refreshing, the shipping fee disappears, the $5.00 is gone and now it looks like shipping is free.
