Order insurance

Hello, I would like to add an option at the checkout which would require the customer to choose "YES" or "NO" to add insurance to their order. If they choose "YES" I want it to automatically calculate insurance at 1.5% of the purchase price (including GST but not including shipping).
Could you please tell me if it's possible to do this?

this method would be good for me, but the problem is I set shipping cost for every product. For example, shipping for the product is $15 and cost of the product is $200.
When I copy my shipping method and set percentage 1.5% - it doesn't use shipping price set for product - it calculates 1.5 from product price only. So I get shipping with insurance costs less than with no insurance.
