Registration page is stretched and some code after date of birth.

Can anyone tell me why the registration page on our side is stretched? See attachment 1.

And after the date of birth (Gebooretedatum) the page shows some code. See attachment 2.

The problem is the date of birth field. If I deactivate this field the registration page is published good.
But if you look at the the edit screen of the Date of birth I can not fill in the standard value.
It shows the same code as on the website. See attachment.

So we now know what the problem is. But how can I solve this?

When I use the advance datepicker of Hikashop the problem is solved.
