How to add the additional fixed cost of delivery?

I'm not following you. You want the customer to be able to select the shipping fee they will pay ? And only if the person is shipping to an island ? Of any country ?
What shipping methods do you already have configured on your shop ?

So why would there be a dropdown or checkbox for that ? Don't you want to enforce the fee based on the shipping address of the user ? Because otherwise, the person can select whatever he wants and not pay for the extra fee even if he is in an island, no ?
In that case, the question would be how do you know that someone is on an island ? Can you base yourself on the post code of the user ? Or the city name ? Or something else ?

In that case, I would recommend using this shipping plugin:
brainforge.co.uk/shop/product/5-plg_hikashopshipping_bfmanual
With it, you can create 3 shipping methods:
- one with the list of the post codes of islands and limited to the Croatia country zone and the shipping fee you want for them in the "price" setting
- one with the list of the other codes (you can use regex to have ranges so you don't have to enter all of them) and limited to the Croatia country zone and the shipping fee you want for people in Croatia, not on Islands
- one with a shipping zone with all the other countries as sub zones and the shipping fee you want for people outside Croatia.
That way, the shipping method will be automatically selected on the checkout based on the shipping address of the customer and the corresponding fee will be charged.
