Adding Hidden Fields

How can we add hidden fields to a form ( vazprep.edu.jm/registration.html )? We would like the purchaser to add further information if required (eg. another Name and Address fields), but those additional fields should remain hidden or a button that would expose/add those additional fields (eg. as in this forum, where the Add File button exposes another field).

You have to create custom fields via the menu Display > Custom fields.

Then it require some css to hide the desired fields

And finally it will require some JavaScript in order to display the desired custom fields.
So it require CSS and Javascript knowledges.
