add a module for additional information on checkou

Hi All,
i need to create a module into the checkout process in order to have more information about my customers, because i sell a service on my website and not product.

How can i do this?

Hi,

To have more informations about the customer, you can add custom order fields.

Else to have a module, you can create a menu then assign modules on this module, and finally use the option "Force a menu on checkout" in Configuration > Checkout

thanks for your reply.
I've added new field in the order; now i have my form.
But i need one thing:
I added a date field in the form, but I would like that some days are "not available" (For example: 21.02.2015 - 06.22.2015) How can I do?
Obviously, time by time, i will need to update this dates.
