Registration missing on checkout

-- HikaShop version -- : 2.2.2
-- Joomla version -- : 2.5.14
-- PHP version -- : 5.3
I have installed this morning the latest version of Hikashop starter and essential and there was problem on the connection page. Everything dissaeared except the login module. It is now impossible for a new user to register (see attached document).

Can you please tell me how to restablish the module?

Can you provide us a screenshot of your HikaShop configuration ? (tab "checkout", section "login & registration").
