Custom Fields of Item table are not displayed on checkout page.

Hi there,
I have created the custom field in item table, but its not getting displayed on checkout page in cart listing.
I have attached the screenshot. I want allow customer to add custom message for each product which he/she has added into the cart.
And then get saved with that respective order.

I have enabled the show fields from item table and show add to cart button for free products in configuration.
Please see screenshot of the custom field settings.

if you want show custom field in checkout page,you should create custom field in 'order' table .

I tried with order table, but it creates only one custom field for entire cart.
I want individual field for ech product in cart. As show in screenshot above.

when create custom field,you can use Restrictions Products in custom field.

If you has 4 product.you must be create 4 custom field and Restrictions Products for per product.
Ex: for Product one create custom field in "order" table (name: info product one) and set Restrictions Products only on Product one
So any product that is added to the cart in checkout page will have a custom field as well.
in end you need css to design simillar your screnshot

I think you are not getting it.
There can be n number of products in cart, so it has to be dynamic.
NOW its four product so you are telling me to create 4 custom fields, so basically what you mean is i should create n number of fields every-time when order is created.

I will be able to do that, but please provide me some instructions to do so.
Basically on checkout page all items are displayed from cart with all its details like name, price, quantity, etc. Along with that i want to display custom TextArea for each product in cart. So that customer can provide comment/suggestions for each product they have added in cart.
I have attached screenshot, how cart items should be displayed.
