Input field on Product Item - how?

I offer personalised gifts and I cannot find a way of offering a text field for the customer to type in on the product item. example: Product A is £1 and has your name engraved on it. Where can I get them to type in their name. I hope that makes sense - many thanks.

It was the table type "item" that sorted it, thank you very much.
