how to add a text area for a product for customer to add customization notes.

customer can select various options for each category. one of the option is Customized option. I want to add a text area above price with options. so that customer can enter customization notes. Also when customization is selected, I want to hide the price so that the customized price can be provided to customer based on his customization.
