Attaching option pdf dowload to selected products

Hi,
In our online Pharmacy we sell some products that can only be issued over the counter (e.g. cod and pickup). This is because the pharmacist is legally required to discuss certain health issues before dispensing some drugs.

Our process would be sped up if we could provide the related questions as online forms at the time of sale. These could be submitted or emailed to let us know if there are any major issues or circumstances that need to be discussed in private rather than in front of other customers.

We are looking for the best way to do this that prompts the user to, optionally, download or complete the form as part of the purchase to checkout workflow.

Also we'd need a way to roll this out using mass actions or something similar because there are so many products (8000) and a range of question sets. However, we do have codes and keywords that uniquely identify the different groups and products.
