Address Problems

I have the workflow set up to ask for the address during login/right after but the modal window to add the address information.
Attached is a screenshot. Why is this?

Hello,

I think that it might be because of your template. Could you try to use another front-end template and tell me if it works ?

If not, you should activate the debug mode of the config of joomla and try again. That might display an error message about the problem.

Thank you for the reply. I've tried the templates that come with Joomla and still have the same problem. Turning on debugging didn't come up with anything either.
Is there anyway to just have the address information on the main page and not in a popup modal window?

Ok, can you show me a screenshot of your customfields listing ?

Did a newer version just get put up? Reinstalling it solved the problem. Thanks!
