Allow packaging and a separate postage cost

Hi,
I would like some advice on setting up the checkout.
I would like to enable the customer to upload a pdf document at the checkout.
I would also like to be able to charge a packaging cost ($15 for up to 5 items and then an additional $3 per item for more than 5 items.
I then want to have a separate postage cost.

Is this possible? I'm not sure how to set up a packaging cost and a postage cost.

About the upload feature you have to set a custom field in order to allow you client to upload a file during the checkout.
For the packaging option the is to use the manual shipping method and set it according to your needs.
You can play with the weight of your products to restrict your shipping methods to specific case.
For example, if all your product have a 1kg weight, you can create a shipping method which is displayed for a 3 to 4kg order with a specific price.
