Prompt user for file upload

We want to offer custom products where the customer will provide artwork.
Is there a way to prompt the customer to upload a file as part of the order process?
There are plenty of file upload extensions, but we would prefer to tie the artwork to a given customer, order and item.

It's actually possible with Hikashop.
You just have to create a new custom field on the table 'item' with the type "image" and make it visible on the front-end.
