User Entry form for product

So, I know for sure that I have everything I need here. We purchased the business version of HikaShop, so we have product custom fields and everything. Here is a picture of what I am trying to accomplish here.

I just want my custom form fields to show up on the product page and I can't get it to work for the life of me. I have tried messing with the options, with the characteristics, I went in and tinkered with the configuration settings, the whole 9 yards. Someone please help me get this figured out.

If you want to use a custom filed which is showing through your product page, you'll have to set the "table" of your custom field to Item.
