Shipment zones and distance

Interesting problem:
My client want an interesting shipping method. I already told him that it might not be possible, but here it is:
Their clients have two options: Pickup or delivery

For pickup, i can find some answers on this forum, but for delivery it needs:
Shipment within 5 kilometers and/or more than € 150,- is free.
Below € 150,- is € 10,-
For every additional 5 kilometers is an extra € 2,50
For shipment outside this provence the delivery goes to a local courier.

Any thoughts on this?

Distance restriction are not available through shipping methods so you can eventually :
- Create some zone which will contain all the zone which are 5km around your warehouse, then create another for all the zone which are 10km around your warehouse...
- Create a shipping method for each zone and set different prices and restrictions.

I'll tell my client that it will be a bit difficult to realize. We have a plan B.
